As a nurse, Coffy (Pam Grier) has seen the ill effects of drugs up close, but it isn't until her little sister becomes addicted to heroin that she finally decides to wage a one-woman battle. Disguised as a prostitute, Coffy goes on a killing rampage, at first going after street dealers and pimps such as sleazy King George (Robert DoQui), then gradually working her way up to bigger honchos. She's shocked, however, when she discovers that her politician boyfriend (Booker Bradshaw) is involved.

One night outside a Los Angeles discotheque, Coffin, a voluptuous woman known as “Coffy,” meets drug dealer Sugarman and offers him sex in exchange for drugs. Coffy and Sugarman drive to heroin addict Grover’s apartment to complete the deal, but in the middle of her seduction, Coffy grabs a sawed-off shotgun from her purse and shoots Sugarman in the face. After explaining to a shocked Grover that she is avenging her eleven-year-old sister LuBelle, who became addicted to drugs, she injects Grover with an overdose of heroin. Later, at the hospital where she works as a nurse, a nervous, exhausted Coffy drops a scalpel during an operation and is sent away by the annoyed surgeon. Before leaving the hospital, Coffy encounters Carter and “Mac” McHenry, uniformed policemen investigating Sugarman’s murder.
